Our goal is to help seniors stay healthier and more independent by improving health outcomes and quality of life. We are committed to supporting the well-being of the communities we serve through personalized, coordinated care.

Picking a Medicare plan? Pick your doctor too.

A plan is only as good as the doctors you can see with it. Before you decide, ask yourself: Is there a doctor's office close to my home? Is my plan accepted there? Can I get most of my care in one place?

We can help you answer the first two in one phone call.

“Went with my mother for the first time yesterday. Everyone, from the lady at front desk, the doctor's assistant Lulu, Dr. Bello and their coordinator extraordinaire Yeni Lorenzo were fantastic! They dedicated plenty of time to make sure they got my mother's medical history and needs in detail. Glad we changed to Cano!”

“The office staff is very professional and friendly. Dr. Olivera and his nurse, Damaris, are incredible. Dr. Olivera took the time to clearly explain my diagnosis and answered all my questions. He is a true health professional, a family doctor anyone would love to have. The place is a bit small but clean and welcoming.”
